i included locations as well as suggested levels for story and explorable modes

A nice list, thank you :) Knowing the minimum level is very useful.

A couple things to note also is that when you get to the minimum level for story mode, you are sent a mail that gives you a bit of back story about the dungeons and, more importantly, also has a clickable link to show you its location. Secondly, the dungeon icon is visible on your map permanently, but it can be hard to remember which is which.

It may also be good to know that there are a set of mini-dungeons. These are non-instanced and often soloable if you want the achievement or duo-able if you want the loot.

I highly enjoyed Vexa's Lab and it was very much soloable.
